Pine Bush partners with Garnet Health for Pfizer vaccination clinic May 20, June 10

The Pine Bush Central School District, in partnership with Garnet Health, isA small vial of vaccine that says SARS CoV-2 with another vial with a needle in it. offering the Pfizer vaccination at Pine Bush High School to any student 12 years of age and older.

The first vaccination will be administered on Thursday, May 20, 2021; the second is scheduled for Thursday, June 10, 2021.

If you would like your child to receive the vaccine:

The consent forms may also be found at the main office of your child’s school.

The consent form must be completed and returned to school no later than Monday, May 17. If your child is learning virtually, please scan and return the completed form to consentform@pinebushschools.org. The original must be brought to the appointment.

No insurance information is needed.

Please note: Students 12-15 years of age must:

  1. be accompanied by a parent; and
  2. sign up ahead of time. No walk in appointments will be allowed.

9 – 10 a.m.: Circleville Elementary, Pakanasink Elementary and Circleville Middle School Students Ages 12-15

10 – 11 a.m.: EJ Russell Elementary, Pine Bush Elementary and Crispell Middle School Students Ages 12-15

11 a.m. – 12 p.m.: Pine Bush High School Students Under 16 Years of Age

12 – 1 p.m.: Pine Bush High School Students Ages 16+
