If you do not have a printer, you may request the forms from your school office.
To be signed and returned:
Permission to Administer Medication Form (if applicable)
Transportation Request Form (if applicable)
Student Publicity Restriction Form (Only complete if you DO NOT want your child photographed for district publicity)
Military Recruitment/Institute of Higher Learning Opt-Out Form (High School only)
