Dear Parent or Guardian:

The Pine Bush Central School District has agreed to assist the Tri-County Prevention Coalition by participating in the 2021-2022 NYS Youth Development Survey which is funded by the New York State Office of Addiction Services and Supports (OASAS).  Students in Grades 8, 10 and 12 will participate in this statewide survey of middle and high school students to help improve our community’s understanding of our students’ strengths and the risks they may face for developing health problems like smoking, underage drinking and other substance use. The survey is totally anonymous with no identifying information collected.  The information the students provide will help us all:   students, parents, schools and youth service providers, work together to strengthen our community and support the healthy development of our children.  

Your child’s classroom may be selected to participate in this survey. The following are facts about the survey:

1. It is totally voluntary. Your child does not have to participate. You do not have to give a reason for not approving participation. 
2. Students are not required to answer all questions, only those that they want to answer.
3. It is totally anonymous. The survey is designed to protect your child’s privacy. No names or any other identifying information will ever be recorded. 
4. The Pine Bush Central School District and its partners will use the school district level survey results to plan better youth development and prevention efforts.

A copy of the 2021-2022 Youth Development Survey is available on your child’s school’s link on our website. You may also call Amy Brockner at 845-744-4011 for more information about the survey.

If you approve of your child’s participation, you do not need to take any action. 

If you do not want your child to participate, contact the person below via email by May 13, 2022.  Your child will be given an alternate activity when the survey is given on May 16, 2022.

1. Es totalmente voluntaria. Su hijo no tiene que participar. No tiene que dar una razón para no aprobar la participación.
